Homework: Public/Community Health Experiential Nursing Learning Course- Community Health Promotion Poster
Following the completion of the volunteer hours, the student will create a poster presentation that would support health promotion for the population served by the agency where the volunteer hours were completed. Part of this homework requires you to complete 11 community health volunteer hours in your home communities, that you are serving as a volunteer, not necessarily a nurse role. The poster will be uploaded into the homework box in the course and in the D2L Discussion area per the date on the course calendar. The week following, the student will engage in discussion with classmates, commenting on the overall volunteer experience and the proposed health promotion plan. There are points for the poster presentation and the discussion points. Please see directions and rubric below for more information.
Outcomes: This homework helps to meet the following student learning outcomes and the end of program student learning outcomes:
I. Participate in interprofessional collaboration, and public health interventions, including health promotion, health education, teaching, screening, illness prevention, and follow-up.
II. Evaluate lifelong learning, informatics/technology, and research evidence to promote population health and nursing actions to guide health promotion, prevention, maintenance, and restoration activities.
III. Value social determinants of health in the nursing process application to provide person-centered care.
Directions:
Following the volunteer hours, the student will create a poster presentation introducing the agency and a public health issue of interest or need related to health promotion or health teaching that affects the health of the community served by the agency. A template has been provided for you.
1. Identify the agency, including the purpose of the agency, the location, and the activity the student engaged in.
2. Identify the population/community served by the agency. Include demographics and description of the population
3. Select a Determinant of Health, health need of the population.
4. Link to a Healthy People 2030 objective
5. Identify a health promotion need and provide realistic interventions on how the nurse could meet the need.
6. Provide collaborative community partners the nurse would work with, including contact information.
7. Identify resources (physical and financial) that would be needed to initiate the health promotion program.
8. Provide outcome measures that could be used to evaluate effectiveness of program.
9. Points will be awarded for creativity and accuracy.
10. Provide reverences.
