Define the program's objectives and identify the outcomes that you want to measure. Design a questionnaire to survey at least five (5) relevant stakeholders to establish baseline data for the program. If you are using the scenario you may use friends, peers or classmates to complete the surveys (you will need to inform these people of the scenario). Include at least ten (10) questions in your questionnaire and state who the stakeholders were and their role in the program.
