Patient: 67-year-old female suffering from watery diarrhea for two days. She did not experience abdominal pain or cramping, nausea, fever, or blood or mucus in the stool. Examination showed a temperature of 36.7 C, irregular pulse of 120 beats per minute, respiratory rate of 18 breaths per minute, and blood pressure of 140/85 mmHg. Examination of the abdominal region was normal, soft and non- tender. White blood cell count was 6.1x 10^9 per L with a slight left shift of 82% neutrophils. Recently returned from Florida where the patient ate fresh oysters. Two days later, the patient developed symptoms. Occult blood was found in stool samples. Clostridium difficile and parasites were not found. Stool samples were streaked onto a CAMPY blood agar plate and t colonies were observed with a mucoid, off-white appearance showing beta hemolysis.
Gram stain: curved gram-negative bacilli.
Strongly oxidase positive.
Grows in 1% NaCl media.
Sucrose fermentation: negative
Ferments glucose
Nitrate to nitrite: positive
Lysine decarboxylase: positive
Ornithine decarboxylase: variable
Arginine decarboxylase: positive
Citrate: positive
Indole: Positive
Lactose fermentation: positive
