Question - Identify the elements of SOAP charting for the following scenario: A patient has come to see the doctor complaining of asthma. She says she has been wheezing and coughing for two days. The patient's vital signs are pulse-102, BP- 146/100, respirations-28, and T-98.6. Following examination, the physician determines that the patient is having an asthma attack. He prescribes the patient a breathing treatment using the nebulizer and 2.5 mg of albuterol sulfate. He had the patient use a nebulizer available in the clinic and then took her vitals again, which were normal.
