CASE STUDY 1: MR. TRAM NGUYEN; CURRENT DIAGNOSIS - SEVERE HYPERTENSION
Nguyen is a 60-year-old truck driver from Bendigo in Victoria. He presented to the hospital three days ago complaining of severe headache and chest pain that started when he was at work, and he was rushed to the hospital. The doctor diagnosed him with Grade 3 (severe) hypertension as his blood pressure readings had been over 180/110 on three different occasions.
Medical history: Type 2 Diabetes, High cholesterol, Chronic back pain (Nil Allergies)
Family History: Nguyen's father died of myocardial infarction at 60 years of age, and his mother suffers from hypertension and Type 2 diabetes mellitus.
Social History: Nguyen lives with his wife and three daughters he has been smoking since the age of 15. and takes alcohol.
Identify and discuss the signs and symptoms of severe hypertension. Discuss the long-term complications of hypertension.
CASE STUDY 2: MRS. SARAH EDRIS: CURRENT DIAGNOSIS - HYPERGLYCEMIA
Sarah is a 55-year-old primary school teacher, married with two adult children. She presented to the ED complaining of fatigue. polyphagia, polydipsia, polyuria, excessive weight loss in the last two months. The nurse noted that Sarah was complaining of blurred vision and a wound that had taken more than 3 months to heal and her BGL was 18 mmol/L.
Lifestyle: Sedentary job, low physical activity level, a diet high in carbohydrates and fats, low in fruits and vegetables and he reports having high stress levels due to work and her diabetes has not been well managed. Sarah is a non-smoker and she does not drink alcohol.
QUESTIONS
1. Discuss the causes of type 2 diabetes. Include non-modifiable and modifiable risk factors.
2. Identify and discuss the signs and symptoms of hyperglycemia.
3. Briefly discuss the chronic complications of diabetes mellitus.
