Identify short-and long-term goals that you will be discussing with the client. You will be checking in on them weekly after you deliver your educational session (weeks 8-10) and Week 10 you will submit your evaluation. The teaching plan should be based on the client's style and preference for learning. Use at least 3 current references to support the evidence base for your plan (client goals, specific strategies to achieve goals, teaching plan, etc..) Include any materials and resources you will use. In Week 10 you will submit your Evaluation of your teaching plan and the participant's progress. Within this chart, you will also create a set of 5 questions you will ask your participant prior to your teaching session and after your teaching session. The purpose is to assess the efficacy of your teaching. These questions assess the knowledge of your participant.
