Evelyn Riley, a 38-year-old female, came to the Nurse Practitioner clinic today complaining of a sore throat, cough, and head pain. You perform the following assessment:

• Onset: Evelyn tells you she began "getting sick" after caring for her grandson Ryan, who had a "terrible cold." She has had a cold and has been feeling run down for a couple of weeks. The sore throat and coughing started two days ago, and her head has been hurting.
• Location: Her head pain is centered over her forehead, and her jaws and upper teeth hurt as well. Her throat feels sore all over.
• Characteristics: Her head pain feels tender when she touches the area, it hurts to chew food, and her head throbs as she bends over. Her throat hurts when she swallows, and she has a nasty taste in her mouth like "dirty gym socks." She also says her nose is stuffy and the drainage is yellow looking. She denies any fever.
• Aggravating factors: Her pain is worse in the morning, and any strenuous activity makes her head hurt worse.
• Alleviating factors: A hot shower makes her head feel better, and popsicles have helped her throat pain. She has tried cough medicine for the cough, but it doesn't really help much.
• Other background information: Evelyn is a single mom in good health. She has three children (two who live at home). She works full time and spends much of her free time caring for her children and her grandson Ryan. Ryan's mother is in an abusive relationship. Ryan is frequently sick and looks undernourished.

You are precepting a student nurse today who is taking her assessment class. She is learning about focused history and assessment skills. In your paper, address the following items:

• In a detailed explanation describing what you would say to the student, explaining the assessment conducted and the findings. Discuss the head, neck, nose, mouth, and throat assessment you performed, and any additional assessments you would perform. What are other factors related to the probable diagnosis you would be concerned about with this client?
• how you would document your findings in the medical record.
• Develop your plan of care. Identify one to two nursing diagnoses and one to two nursing interventions related to those diagnoses. The interventions should be evidence based. Cite the references used in your plan in correct format.
• Identify what client education should be provided to Evelyn, given her background and presenting illness. Describe your teaching strategy and how you will evaluate the effectiveness of the educational intervention.
