Hospital Discharge Plan-

SCENARIO
******Tina has been hospitalized for four days. She has been afebrile for the last two days. Her PICC line has been removed. The discharge planner lets you know that Tina has home healthcare for diabetic education and management arranged for discharge today. Her antibiotics will be by mouth. She will give herself insulin. Her mother knows how to change the wound dressing, and the home health nurse can do more training if needed. Tina will also be tracking her blood sugar at home with finger stick glucose testing, and she has all the supplies at home. Tina has agreed to take her medications as ordered and has the money to get her prescriptions filled. She will be discharged according to the provider's orders, and you need to teach Tina what she needs to know before she goes home.

Instructions

Write all the important information you need to teach Tina on the topics below before she is discharged from Shadow General Hospital. Provide guidance for diet, activity, diagnosis, medications, blood sugar monitoring, wound care, and follow up. Justify the need for the education provided and include ways to prevent worsening of disease states. Remember to write your recommendations as if speaking directly with Tina Jones.

Write what you would say to instruct Tina about managing her diabetes.

Asthma
Direct to keep inhaler available at all times and to avoid known triggers. She should follow up with her primary care provider about managing her asthma long term.

Educate Tina about avoiding situations that could cause an asthma exacerbation. Explain to Tina that she should manage her asthma with her primary care provider.

Dehydration
Direct to drink 8 cups of water per day in order to ensure adequate hydration. Medications should be taken with a full glass of water.
Explain to Tina why it is important to stay hydrated

Wound Infection, Right Foot
Direct to keep the wound clean by changing the dressing as ordered.
Educate Tina on her condition, management, and potential environmental and lifestyle changes. Explain to Tina that she should take all antibiotics as directed and until gone.

Wound Care
Change wound dressing twice daily. Tina's mother requests instructions on how to change the dressing before discharge.

Medications
• For asthma: Albuterol MDI 2 puffs every four hours as needed. Use as needed for cough, wheezing, and shortness of breath. • For diabetes: Insulin glargine 10 units subcutaneous at bedtime • For diabetes: Metformin 850 mg by mouth twice a day. It may cause gas and loose stools, which should improve over time. • For infection: Linezolid 600 mg PO Q12hours for 10 days. • For pain: Oxycodone 5 mg/Acetaminophen 500 mg by mouth three times a day as needed for pain. Only take as prescribed. If pain is not well controlled, call primary care provider. Educate about fall risk associated with narcotic medication.
Educate Tina about her medications including the purpose and the side effects of each. All written instructions and prescriptions should be in layman's terms.

Diet
ADA 1800 calorie diet. Encourage reading food labels for carbohydrate count. Home health follow up with a diabetes educator. Use the suggested caloric intake to encourage reading food labels. Emphasize that lifestyle changes can improve diabetes outcomes.
Educate Tina about proper nutrition practices for health promotion and diabetes management.

Follow Up

When to Return to ER or Call Primary Care
Return to the ER or call primary care provider if patient develops fever of > 100.4° F, chills, pain worsens, or any new redness or swelling. The signs and symptoms of hypoglycemia are shakiness, confusion, sweating, anxiety, hunger, blurred vision, seizures, and loss of consciousness. Eat candy, drink juice, or take glucose tablets. Call the medical doctor if unresolved. Call primary care provider with any questions.

Provide Tina with information to identify occasions when she should call the ER or her primary care doctor.

Whatever was done in the scenario and instructions of the HOSPITAL DISCHARGE PLAN needs to be incorporated in the signed as per the rubric. Although the QUESTIONS for The scenario and instructions are different, The scenario and instruction ANSWERS are the same unfortunately I needed to include them because they were all together as the assignment was one.. When incorporating information on her education in regards to her discharge...references need to be inched up to four whether on for ex diabetes, asthma i need to complete the hospital discharge assignment. Now i want to know experts response to each of the challenge is given, so what exactly is required as the responses are pretty much the answers to the questions which would not vary much from my opinion as well, so what needs to be done exactly and how to cite and provide reference for an already written response.
