Use these data sources to provide a well-cited justification for choosing the leverage point at which the program will intervene to address childhood obesity in Jefferson County, Mississippi.

Phase 4: Intervention Plan Development Develop the intervention plan to address the leverage point identified in Phase 3: List one overall program goal the intervention aims to achieve. List three SMART objectives that, when achieved, will logically result in attainment of the program goal. Describe the intervention activities that will lead to achieving each of the three SMART objectives. Describe how stakeholders will specifically be involved in the implementation of the intervention activities. Also describe how the unique cultural values and practices of the community that might affect program acceptability or success within the priority population will be considered.

Phase 5: Evaluation Describe how the program will evaluate each of the three SMART objectives listed in Phase 4 to determine if they were each achieved. Describe how stakeholders will specifically be involved in the evaluation process. Describe how the findings of the evaluation will be used.
