The team should include representatives from various sectors, including people with disabilities, healthcare providers, disability advocates, policymakers, and community-based organizations.
The second step would be to develop a vision for the program. This involves creating a shared understanding of what the program hopes to achieve. The vision should be developed with the input of people with disabilities and other stakeholders.
The third step would be to conduct a community health assessment. This involves gathering data on the health status of people with disabilities in Kentucky, including their access to healthcare, prevalence of chronic conditions, and quality of life.
