Assessment 1 - Burden of Chronic Conditions
The topic and your task:
PART ONE: Choose a chronic condition contributing to the burden of disease.
Examples of chronic conditions to choose from include:
a. Arthritis
b. Diabetes
c. Anxiety and depression
d. Chronic kidney disease
The population you select needs to be from ONE of the following:
• Young Australian adolescents
• Elderly (65years and over) Australians
• Aboriginal and Torres Strait Islander populations in Australia
• Migrants/Refugees in Australia
• People living in rural and remote regions of Australia.
Use relevant databases and websites to locate journal articles, statistics, and data about the burden, prevalence, public health significance, burden of treatment, risk factors, and the broad range of determinants of the chronic condition.
Choose a framework commonly used for the prevention or management of chronic conditions and critically review its application to your chosen chronic condition.
PART TWO: A recorded PowerPoint presentation (7 to 8 minutes and 8 slides max) to be presented to the Management of a public health organisation.
You MUST SUBMIT BOTH PARTS of the assessment to receive a mark. Otherwise, your submission will not be assessed.

• To identify key determinants surrounding the development and significance of a major chronic health condition.
• To critically assess, interpret and synthesise different types of data used to measure the burden of chronic conditions, assess their risk factors and determinants, and their significance at a population level.
• To critically examine a framework recommended for use by an interdisciplinary team for the prevention or management of a chronic health condition.
• To synthesise and interpret relevant readings into a coherent report.
Length:
PART ONE: 1500 words +10% words (excluding diagrams/figures and reference list - in-text referencing is included). No appendices should be included in the assignment. The content of any Table is included in the word count.
PART TWO: Recorded PowerPoint presentation (7 to 8 minutes long and 8 slides max)
Weight:
PART ONE: Font size 11pt or 12pt Times New Roman or Arial, or Calibri 1.5 or double line spacing, Standard 2.5cm margins
PART TWO: You should aim to present each slide in one minute. Do not type in the slides everything you want to say. Your slides must present in a few words/bullet points the main points you wish to cover, and you must elaborate on those with your own words.
PART ONE - Template:
As an academic report, your assignment should have an introduction, a body, and a conclusion. The report should include:
• A title page (Your name and student number, title of unit, title of assignment, unit
coordinator's name, date, total word count)
• A rationale and public health significance of your chosen health issue as a chronic condition
• The burden of the chronic condition both globally (i.e., worldwide) and locally (e.g., Australia)
• Most significant three broad determinants (e.g., environmental, social) of the chronic condition
• Most significant three individual risk factors for the condition
• The burden of treatment on your chosen chronic health condition
• A description of a framework commonly used for the prevention OR management of chronic health conditions.
• A critical review of that framework when it is applied to the prevention OR management of your chosen chronic condition.
• Current and high-quality references using the APA 7th ed. referencing style3
PART TWO - Template
Your goal is to offer critical insights and lessons from the analysis that could be useful for your organisation.):
