You need to include:
- Pseudonyms (anonymise/, make a different name) for the person/family members (make reference to the Nursing and Midwifery Code(2018 ) and the application to confidentiality). Do not name the placement that you encountered the patient/family.

- Condition/disease they are living with.

- Anything of particular interest about that person or that family
- Are they health literate and is there any barriers to healthcare and/or social services?
Physical
Identify the influences on physical wellbeing to this patient/family.
You need to include: how do they physically function and how dependent or independent they are; You may want to consider relevant activities of living:

- eating and drinking

- cleansing and dressing

Psychological
Identify the influences on psychological wellbeing to this patient/family.
You need to include the spiritual and emotional beliefs and values of the patient/family. This may be:

Discuss the communication and relationship management skills nurses can use to facilitate patient wellbeing in relation to these.
