Promotion
Purpose:
During your experiential nursing learning Immersion II in NURS 450, you will complete an evidence-based community health promotion project focusing on a plan to improve or maintain the health of a priority and targeted/aggregate population. The project cannot be the same community health assessment (windshield survey) you completed in NURS 400. For this homework, you must select a community of interest such as your home demographic, identify a public health [promotion] problem within your priority/targeted aggregate population, analyze epidemiologic data (demographics, census tract summaries, morbidity and mortality data, etc.), and other pertinent findings and evidence to formulate an evidence based community health diagnosis. For this homework you will develop a plan for implementation (including barriers and social and cultural considerations), and minimum of 2-3 public health nursing interventions including an outcome evaluation of your project for the community and aggregate. You will further analyze and identify social determinants of health and health disparities within the aggregate population to support your health promotion topic.
Part of this homework requires you to complete 11 community health volunteer hours in your home communities, that you are serving as a volunteer, not necessarily a nurse role. You must identify and assess a health concern/problem within an identified aggregate population (depending on where you volunteer). Include your 11 volunteer hours as part of your community assessment (windshield survey) for your Evidence-based Community Health Project. You will have to complete a narrated presentation sharing components of the Evidence-based Community Health Project.
Students have volunteered at places like Food Link, homeless shelters, domestic violence shelters, salvation army, etc. In this volunteer role, you must be in direct contact with the clients who come to the location/site/agency for services (i.e., rather than for example you being in the back of the location sorting clothes or food).
Task
Question 1. Choose a public health issue of interest or need related to a health promotion topic that affects the health of your community. You may choose a health promotion topic that is of interest to you and of importance in keeping people healthy in your home or work community. Remember to assess assets and needs in the community to address the health issue through your windshield or walking community health assessment survey.
Question 2. Identify (and cite) a Healthy People 2030 Objective to address the previously identified or newly identified health issue.
Question 3. Identify health outcomes and recommended evidence-based public health nursing intervention(s) to improve health outcomes in this population.
Question 4. Strategize implementation and evaluation of this intervention.
Question 5. Implications of and suggested policy changes or program changes.
Question 6. Provide epidemiologic and logical reasoning for your identified health outcomes and recommended nursing intervention(s).
Question 7. You will create a PowerPoint (or other creative method) to share your health promotion project with your peers via the NURS 450 Discussion board.
