Assessment 1:

Written piece examining a public inquiry into a health system failure Cohort E only 1000 words

Both nationally and internationally, there have been public inquiries into the failure of health systems to deliver safe and evidence based care.

The 2005 investigation into clinical incidents, surgical errors and patient deaths at Bundaberg hospital QLD, highlighted multiple inadequacies of clinical governance, leadership and behaviour of staff. The surgeon Dr Jayant Patel was appointed surgical director for the hospital during a period of time where the hospitals goals related to increasing the number of patients treated. This is a well published example of an Australian health system failure.

This assessment requires you to explore this prominent inquiry and to answer the question posed in two only of the four modules below;
• Clinical Governance
• Quality & Safety
• Leadership and Management
• Organisational Culture

A brief introductory paragraph is required to identify which two modules you will address in your response. This introduction is not included in the word count. As a written assessment, please use a third person writing style.

Your response for each module will be 500 words (1000 words in total).

A brief list of resources have been provided in the assessment section of the LMS to assist you in beginning to explore the context of this health service inquiry. You must support your assertions with evidence from additional sources of contemporary literature.

1. Clinical Governance
Clinical governance is defined in the Victorian Clinical Governance framework (2017) as:
"..the integrated systems, processes, leadership and culture that are at the core of providing safe, effective, accountable and person-centered healthcare underpinned by continuous improvement.

There are five domains in this refreshed clinical governance framework. Within the five domains, key systems and practices are required to support safe, effective, person-centred care for every consumer, every time.

From the model above, identify two domains. Describe how these two domains may have influenced the system failure in the Bundaberg hospital public inquiry.

Use the literature to support your assertions.

2. Quality and Safety

Identify and describe a risk management strategy that the organisation in your inquiry could have undertaken to prevent the health system failure.

Such strategies may include:
• Credentialing
• Reporting and acting upon near misses and incidents
• Accreditation and benchmarking
• Auditing
• Monitoring and responding to complaints
How might the implementation of this risk management strategy have changed the outcome in the inquiry?

3. Leadership and management

Identify and describe two traits/characteristics of an open and transparent leader.
Were the characteristics evident in the key figures in the inquiry? Use the evidence from the inquiry to support this assertion.
Briefly examine how these characteristics influence the delivery of safe patient care.
Use evidence from contemporary literature to support your assertions.

4. Organisational culture
Define the term Whistle-blower.
Describe what influence the organisational culture had on the quality of care in the inquiry?
Did the culture of the organisation help to hide or disclose the failure? Justify your response with evidence from the literature.

Assignment 2:
(2000 words)

This assessment is divided into 3 parts.

1. Critique contemporary nursing leadership theories, core competencies and personal capabilities and their importance to individuals, teams and the nursing profession.

2. Develop and apply communication skills that directly relate to health information systems.

3. Apply awareness of the transition from student to registered health care practitioner through critical self-reflection

You will be required to reflect upon your own experiences to date and to search for evidence from the literature to address questions regarding contemporary professional practice issues.

At least 18 relevant peer- reviewed sources are required. Other sources such as credible websites may be used to add to your reference list.

The assessment parts in GREEN text, require you to explore the literature to support your answer.

The assessment parts in BLUE text, require you to reflect upon your experiences in, and knowledge of, the health care sector. You should also support your opinion and assertions with evidence from the literature for these responses.

Introductory and concluding paragraphs are not required but each question must be answered in full.

As the blue task is reflective, it is appropriate to use a first person writing style.

Leadership and Management (650 words)

This module requires you to complete the Leadership Skills Inventory (on LMS) and reflect upon your result.

In light of your results, please answer the following:
• Identify and explore two key leadership skills that Registered Nurses must bring to their role. Use evidence from the literature to support why these skills are important.

• As a result of your inventory, describe up to two skills you would like to further develop and identify the strategies that you might use to achieve this. Use evidence from the literature to support your assertions.

Communication and Health Informatics (650 words)

There are multiple sources of patient data that are collected and recorded across primary, secondary and or tertiary health services. In this module you are required to:

• Choose a particular service area (e.g. rehab. aged care, theatre, outpatient service) and describe key challenges that nurses face when utilising the medical record by outlining aspects of confidentiality, privacy and safety.

• Consider the broader topic of digital health in planning patient care. Describe innovations that you consider could enhance your practice and delivery of person centred care.

You should include a discussion topic of interest here related to implementation of new technologies such as robot receptionist, huggable robot or other emerging technologies.

Professional practice

There are many potential sources of stress when working as a registered nurse. Developing and implementing coping strategies to manage stress is an important skill for all nurses.

In this module you are required to;

- Define one of the following terms - role conflict OR role ambiguity. Discuss the impact that the chosen concept may have on a registered nurse (RN).
- Identify and describe at least two self-care strategies that you will employ to facilitate you staying emotionally and physically well in your new role as a RN.
- Describe two positive work place behaviours or attitudes you will practice in your graduate nurse year and explain why these are relevant to your goals.

Use evidence from the literature to support the effectiveness of these strategies.

Assessment 3 - Written reflective piece addressing contemporary practice issues

1. Identify and discuss all the clinical practice issues of concern in Daisy's care during her hospitalisation. Your discussion should include
a. examples from the case study
b. identify which standards have been breached
c. link how those practice issues demonstrate a breach to the National Health and Safety Standards (word limit for this section is approximately 500 words)

2. From the list of clinical practice issues that you have identified in Question 1 above, discuss

a. the 4 top priority issues that require attention in Daisy's care

b. recommendations related to the priority issues you would make to staff/NUM for changes to health care practice generally at the ward level to ensure a similar situation does not happen with other patients. Provide rationales for your recommendations. Recommendations and rationales are to be supported by evidence-based literature.

Part B

Step 1 - Follow the guidelines in Section 1 Poster Development.
Step 2 - Follow the guidelines in Section 2 Oral Presentation of your Poster Section 1 - Poster Development In Case Study Part A, you identified, and discussed how the clinical practice issues experienced by Daisy Mayes and her family were linked to breaches of the National Health and Safety Standards. There are quite a number of National Health and Safety Standards that were breached in her care.
For the purpose of this assessment you are asked to *choose only one high priority National Health and Safety standard that was breached in Daisy's care and recommend practice improvement changes that could be implemented to ensure that a similar situation does not occur in the future for other patients on the ward. *Please note: Your recommendations for change must be supported by peer-reviewed evidence based literature.
Structure of Poster (Approximately 300 words)
Your poster should include the following:
• Title The name of the chosen National Health and Safety Standard you will be addressing.
• Introduction Provide a statement which summarises Daisy's case and link how the chosen Standard was breached. This will help to set the scene for the rest of the poster.
• Key Sections Include several key sections in which your topic is to be reported (Some examples, of key sections to be included in your poster are: o clinical issue being discussed o Discussion of what the literature states is best practice for the clinical issue o Recommendations for Change supported by peer-reviewed evidence-based literature
• Reference List (this can be printed on the back of your poster).
Section 2 Oral presentation of your poster (10 minute presentation) Oral Presentation Instructions
1. Students will display their poster on the classroom wall.
2. Student is to deliver a 10 minute oral presentation of their poster to the class and allow time for questions from the audience. Time for this activity is allocated as follows:
• Oral Presentation = 5 minutes
• Questions from audience = 5 minutes 3. The oral presentation should expand the information in your poster as follows:
• provide a brief explanation of the chosen standard
• state how the Standard was breached in Daisy's case
• explain current best practice, i.e. what is the best practice recommended by the Standard
• explain in more detail the recommendations and a rationale
• elicit feedback/questions from your audience.
4. A presentation schedule will be displayed on the classroom wall on the day of presentation.
5. Marking rubrics will be supplied by your workshop facilitator at the time of your presentation.
