The Scenario - You are a Safety Officer based at Company Headquarters (in your State/Territory of current employment). You have been asked to conduct some general Hazard Identification inspections and report back to your employer with results.
Upon arrival at the first site, you consult with a HSR who advises that there are reports of serious hazards and risks of injury around accessing a sub floor area in a workshop. The sub floor area has recently been used as storage area and improvised welding booth and there are two 'F' size cylinders of Oxygen stored below, along with four G size acetylene cylinders. The HSR advises you that people use this area because it is out of sight and they do not need to worry about housekeeping issues when clients visit the factory. The HSR states that they have raised four hazard reports at site level and they are being told by supervisors that its being "looked at".
There is no record of how often this area is utilised by employees and there is not currently any injury data associated with this location (including near miss).
You visit the work area and upon arrival, observe a worker exiting the sub floor access point as depicted in the photograph (see in attached file).

When you introduce yourself the employee says, "oh yeah, here we go" and walks away from you rapidly. You secure the location and commence investigations into the apparent work processes.
Upon initial investigation, you identify that no risk assessments have been conducted for this location or work processes. You are further advised that they equipment is kept outside the entrance point "just in case".
Task -
1. Assessment -
Identify and access the WHS appropriate legislation, regulations and codes of conduct related to the scenario. Using this information:
- List the breaches of law you observe.
- List the immediate corrective actions.
2. Briefing -
Write a briefing document for the employer and site management regarding: -
- Their duties relating to safety management on site.
- Specific duties regarding hazard identification.
- Specific duties regarding risk assessment.
3. Consultation and controls -
Using the applicable Act, Regulations, Codes of Practice, Standards and Guidance Materials to:
- Design a risk assessment document identifying hazards.
- Assess the risks associated with each hazard.
Who would you consult with to best manage this scenario? Describe for each stakeholder:
- What you would consult about.
- The method of consultation.
4. Recommendations -
Applying the hierarchy of control to the circumstances and hazard management, list your recommendations regarding this scenario. Ensure you include and explanation of what constitutes Reasonably Practicable at law and how it applies to your recommendations.

Based on your analysis of the investigation, specify your suggested corrective actions.
6. Monitor and Evaluate -
Describe each of the following:
- How you evaluated the effectiveness of your recommendations.

Task 2: Practical Application Task - Part 2
Use the same scenario from Task 1 to complete the following.
1. Recording of Consultative arrangements -
Identify the requirements for consultation relevant to the scenario and write a simple consultation procedure. The procedure must: Satisfy consultative duties as set out by relevant legislation and Be simple enough for another person to follow in the same or similar circumstances. Reference risk assessment arrangements.
2. Communication and Training -
Prepare a staff briefing for a site meeting to educate them on the situation and required actions.
Your briefing must include reference to the relevant Act, Regulations, Codes of Practice, Standards and Guidance Materials.
