a) From the various Australian community health settings, select one (1) Primary Health Care (PHC) Nursing role of interest to you. Describe this PHC nursing role. Include an overview of the practice responsibilities related to their role in the community health setting. As part of your discussion, explain why this is considered a PHC nursing role.

b) Outline your selected PHC nurses' role in health promotion. Use the template provided to identify the Ottawa Charter health promotion strategies and for each include a related activity your selected PHC nurse would undertake as part of their role in the community health setting. Includeyour completedtemplate in your written assessment 1 submission.

c) Choose one (1) consumer group your selected PHC nurse would support as part of their rolein the community health setting. Briefly define and describe the general characteristics for your chosen consumer group. Identify one (1) common health focus/concern/need for chosen consumer group that your selected PHC nurse would support within the community health setting. You will need to include relevant Australian National and State statistics and/or demographic data in your discussion and you need to comment on theimportance of supporting this consumergroup with the identifiedhealth focus/concern/need.

d) Identify a health promotion/education/information resource your selected PHC nurse would use to support the health literacy for your chosen consumer group in relation to the identified health focus/concern/need. Your discussion will include an explanation of the nurse's role in consumer education and health literacy.

e) Critique the selected health promotion/education/information resource using the template provided. Your critique will include feedback from other health staff, nurses and/or consumer group in a relevant clinical setting. Includeyour completed template in your written assessment 1 submission. Bullet points are acceptable in the completion of this critique template.

f) Based on your critique, produce a modified health promotion/education/information resource. You will redevelop this health resource using the information from your critique. This will demonstrateyour applied knowledge and understanding for the health literacy of your identified consumer group and the health focus/concern/need.
