Write a questionnaire for the below hypothesis about ERM system to collect data to why participants were not entering data in ERM.
Hypothesis 1:
The sample size and group sampled did not satisfy the need of the project.
Cause: The small size and the diversity of the clinics sampled.
Effect: Data collected was not enough.
Hypothesis 2:
Physician's poor participation in data entry is linked to data efficiency.
Cause: The lack of data efficiency, which could be the result of not collecting the right data from patients or not running the required tests on patients.
Effect: Limited participation of physicians in entering data to the EMR System.
Hypothesis 3:
EMR system poor efficiency highly effects physician's participation in data entry.
Cause: The poor ability of the EMR system to run efficiently, effectively and smoothly on the user-end.
Effect: Lead the physician's to be less participating in data entry.
