Question

Currently patients who are experiencing hypercapnic respiratory failure requiring noninvasive ventilatory support will be placed on bilevel positive airway pressure (BIPAP) to improve ventilation and perfusion. This form of Noninvasive Positive Pressure Ventilation (NIPPV) uses pressure to decrease the work of breathing and improve the gas exchange on a cellular level (Khan et. al, 2022). Average volume assured pressure support is another mode that can be used to correct this imbalance. In this mode the Inspiratory Positive Airway Pressure or IPAP time is not fixed as opposed to BIPAP. A range instead is used to achieve target tidal volume (Claudett et al., 2022). A serious complication of the use of NIPPV is emesis. Evaluating the development of aspiration pneumonia with these modes could potentially be a deciding factor in which mode is superior.
