Problem
I. List five questions you would ask your client to gather the additional information that would be valuable in making your assessment and program recommendations. For each question, include your rationale for why this extra information is relevant to your assessment.
II. Identify and describe three resources in your city/region that may suit your client's needs and profile. Include a summary of each resource's services and identify how each applies to your client.
III. Provide a detailed description of the referral process and the means by which your client may access your selected resource. Include the following information:

2. Any fee associated with enrollment/admission;
3. Specific actions to be taken by the client;
4. All required information and documentation for enrollment/admission.
IV. Provide a detailed summary of how you would prepare your client to attend the resource. Include the following information:
1. How you would describe the resource to your client?
2. What you would say to encourage your client to attend the resource?
3. Potential barriers to your client's attendance.
4. Solutions to the potential barriers to attendance.
Case
Doug (24) has had a long history of minor convictions, including shoplifting, assault and drug possession. He had been a ward of the CAS until his 16th birthday, when he left his group home and dropped out of high school. He has been in unstable housing and in and out of jobs since that time. Most recently, Doug has been living on the streets or couch-surfing. He wants to change his life and know where to turn.
