A. Describe the problem - why is it a problem? What goes into the problem? Try to be as detailed as possible in describing how it is specific, measurable, and achievable.
B. What are we trying to accomplish (Aim)? How will we know a change is an improvement (Measures)? Identify the stakeholders.
Step II:
A. Identify the area of intervention and design a PDCA cycle plan. Include all the details of your study design, including who, what, where, and how it will be implemented.
B. Discuss how you will establish and track measures of improvement during the "plan" and "do" phase of PDCA.
C. Explain how you plan to increase the scope of subsequent test cycles based on what you are learning during the "Check" and "act" phase of PDCA.
