Writer was able to speak with child's mother, Lucia and provided a brief description of the CMO and the Family Support Organization and what services we can offer to Joseph and the family. Writer scheduled an initial meeting with the family for Thursday 4/5/15 at 3pm in the family's home. Writer informed Lucia that CM, CM Supervisor, and FSO worker would be present, and reminded her that Joseph must also be present for the meeting. Youth's mother confirmed that Joseph would be present. Writer informed mother the initial meeting would involve CM and FSO worker providing further details about services, the creation of a crisis/safety plan, and an explanation of consent to release information forms for the purposes of care coordination. Writer confirmed address and insurance information with mother.
