Principle of palliative care
Mr Chiu is a 48 year old man with a progressive glioblastoma. He has an expressive and receptive dysphasia and finds it increasingly difficult to communicate. He is also aspirating on thin fluids. Functionally, he requires 2 people to transfer, and on his worst days requires the use of a hoist lifter. Weight, gained as a result of his dexamethasone dose, also contributes to this problem. He has had seizures, which have been increasing in frequency of late to one per fortnight. His sleep-wake cycle has been reversed; his mood is often labile and his insight is poor, which makes his care challenging.
