An elderly woman is admitted from her home. She has a hip fracture after falling in her front yard when she tripped. She is now 72 hours post hip fracture repair. You are completing your initial shift assessment. She is now confused and somewhat combative in her behavior. The report you received indicates that she was alert and oriented to person and place. You know that these changes can be from several things.

1. How would you assess this patient? On what areas would you focus? What other assessment findings might change?
2. For what is this patient at risk? What risks are related to age? 
3. What would you expect from the vital signs based on her changes? What will her temperature probably indicate? Her heart rate? Pulse quality? Respiratory rate? Breath sounds? Blood pressure? Pulse oximetry? How will you assure their accuracy? Based on those identified changes, what might be her primary problem?
4. Once you have determined her primary problem, what is her evidence based plan of care? What nursing interventions will you perform? What doctor's orders do you need? 
5. How would your plan of care differ if the patient were only 34 years old?
