Assignment: As explained in the Syllabus, students will construct a Survey Assessment Tool that will help them address the issues/shortcomings they found in their literature review regarding their selected research topic. Therefore, questions should have the ultimate goal of potential providing you with vital information.
First, students will generate a Cover Page that briefly explains the purpose of your study and survey. How will people benefit from participating? Additionally, students need to address how respondents' confidentiality will be maintained should they choose to participate. Also, will you provide compensation for participating? Will there be any follow-up surveys? All of these issues need to be addressed in your Cover Page.
The Survey Assessment Tool must also be designed in the appropriate layout with questions flowing logically from one to the next. For example, if you ask a question about whether or not a person drinks alcohol, an appropriate next question would be "How many times a week do you drink alcohol?"
At a minimum, students will construct a Survey Assessment Tool that incorporates:
• (5) demographic questions.-blue
• (5) matrix questions.-green
• (5) Lickert Scale questions.-red
• (3) contingency questions.-purple
• (2) open-ended questions.-black
Each question will be worth 5 points. Color code your questions as highlighted above.
