The main complaint of a 64-year-old female with a history of chronic obstructive pulmonary disease (COPD) who visited the emergency room (ED) was that his shortness of breath had gotten worse during the previous two days. After a recent upper respiratory infection, the SOB started. His oxygen saturation on room air in the ED is 88%. He is speaking in fragments and appears to be quite exhausted while complaining of acute dyspnea. The following are his vital signs: HR: 132 beats/min RR: 28 breaths/min BP is 151/93 mm Hg. diffuse wheezes and a protracted expiratory phase are the lungs' noises. ABG results for arterial blood reveal a pH of 7.12, a PaCO2 of 78 mm Hg, a (HCO3-) concentration of 32 mEq/L, and a PaO2 of 61 mm Hg.
