General guidelines for writing the report (format):
The cover page should include the university, college, department, course title and code, student's name, ID, academic year, and the Title of the report.
The report should be submitted typed on Word document on BB.
The report must be written in professional 12 fonts times new roman, in double spacing fashion using APA style.
The deadline for report submission will be as indicated in the timetable.
Specific guidelines for writing the report:
State the objective of the clinical practicum, in general, and the specific objectives of each unit of the practicum.
In order to fulfill the requirements to cover each item in each standard, you have to answer the following questions (as applicable):
What is it? (i.e., define it as it is applied in the setting of the practice).
Why is it? (i.e., the purpose of its conduction or presence).
Where is it? (i.e., the place of conduction, presence).
When is it? (i.e., when it is being found or conducted).
Who? (i.e., who is the responsible for conducting it, and to whom it is directed).
How? (i.e., the process of its conduction or utilization).
The reference guidelines (as policies and procedures) should be referred to as necessary in the item discussion.
You have to put your comments (i.e., the strength and weaknesses) in relation to each discussed item, as well as your recommendation to modify it to be more efficient, effective and/or safe to the running system (if needed).
