Select one of the data elements that is utilized in the UHDDS Data Set and discuss your insight on how this data could be incorrect by the time it is submitted to the state data base. There are states that report health statistics for hospitals on a website that consumers can access. Examples of information reports from the database include the number of babies born each year and the number of deaths reported from the hospital. Incorrect data might be a female diagnosis code or surgical procedure code that is being used on a male patient. Besides giving an example of an incorrect data element, provide justification for who in the HIM department should be responsible for submission of data and corrections to the state. In addition, please construct standards to ensure the data is submitted correctly.
