Case: Mr. Rhodes is a 45 year old male seen in the emergency department for right sided abdominal pain, nausea, and vomiting. The patient is diagnosed with appendicitis and scheduled for surgery at 8am the next morning. 

Current Vital Signs

Pulse 103

Blood Pressure 110/78

SpO2 94% on room air

RR 17

Temperature 100.8 F

 

Previous Medical History 

Essential Hypertension

 

 

 

 

 

Current Medications

Lisinopril 10 mg PO QD

Current Orders:

NPO until surgery

Normal Saline 9% at 125 ML/HR x 24 hours

Morphine 2mg IVP Q 4 hours PRN pain 

Rocephin 2g IV X 1 now

Incentive Spirometry use Q 2 hours while awake until 48 hours post-op.

Problem 1:  Mr. Rhodes is admitted to the preoperative suite and asks the nurse for a glass of water.  How should the nurse respond to this request?

 Problem 2:  Mr. Rhodes tells the nurse that he is nervous about the surgery and is wondering what he should expect after his surgery. How should the nurse reply?

Problem 3:  The surgeon just left the patient's room after going over the consent for surgery form and having the patient sign the document. Mr. Rhodes then states to the nurse that he is unsure and still has questions about the procedure.  How should the nurse proceed?

 Problem 4:  The patient complains of 8/10 pain in his right abdomen. After the administration of the ordered morphine, how should the nurse evaluate the effectiveness of the medication?
