Tom is a 28-year-old male with intellectual disabilities and little speech. Tom has recently moved into supported accommodation because his parents could no longer cope with his behaviour. Tom often makes loud repetitive noises, at times he lies on the floor and refuses to get up, and occasionally he hits other clients. A behaviour management team have been assigned to develop a positive person-centred behaviour management plan and an individual response plan for Tom.
After implementing the plan, staff monitored it and at the first review it was noted that Tom was responding well to the strategies to address his repetitive behaviour and that the hitting had stopped, but Tom was still falling to the ground on some occasions. Staff had used a strategy of encouraging Tom to get off the ground by offering him an apple, but that had actually reinforced the falling to the ground behaviour. How might staff modify the plan?
Staff use data and other information to develop an individual response plan for Tom. Identify some active strategies that might be used to address Tom's falling to the ground behaviour.
Identify the least intrusive effective active strategy.
If staff need to use reactive strategies, what do they need to take into account when selecting the strategies?
Identify reactive strategies that maintain Tom's dignity.
On rare occasions, Tom has fallen to the ground in the middle of a shopping centre car park which is very dangerous for him and for staff supporting him. Identify strategies that might be used in a crisis response plan for such a situation, taking into account duty of care, ethical and legal requirements.
How might Tom be supported after the episode?
How can staff monitor and review strategies to determine effectiveness in developing and maintaining positive and adaptive responses, to determine reduction of risk of harm to Tom and others, and to determine level of intrusion on Tom's dignity and self-esteem?
List the documentation staff might need to complete while supporting Tom.
How should the documentation be filed?
