Johnny enters the health care facility and claims he has broken his femur. This is his first time here. Rhonda greets him and lets him know that she will need to gather information from him. Rhonda enters in his demographics, such as his name, address, and insurance information. After Johnny has left the health care facility, a medical coder reviews the physician notes and applies a medical code to his patient record to send to the insurance company.  

Initial Post:  answer all three parts of the questions with good explanation
