Describe an experience where you interacted with a healthcare provider (keep it general, do NOT share private health information) either as a co-worker, patient, parent of a child, advocate, or community member while the provider was providing direct patient care and analyze the interaction. How involved was the patient in the decision process? What feedback would you provide to the provider to improve the patient's experience? Was health information technology (HIT) (e.g., EHR, CDSS, patient portal) and/or a patient health IT navigator used? If so, did this enhance the interaction and improve overall health outcomes?
