Scenario: You have been asked to put together an information pack for service users who want to find out more about the health and social care services available to them. You need to research and write up information about a range of health and social care services available and how individuals can access them. You also need to include how professionals work in partnership to support service users in health and social care. You will use this pack to help Elsie decide which services might be useful for her. Elsie is an 87 year old lady who is elderly and has dementia. Elsie lives in her home in a remote part of Clee Hill but has recently become physically disabled after a fall, meaning she can no longer walk without help. Elsie is profoundly deaf. She has decided she needs to live somewhere she can be looked after but has only her state pension to live off.
