Sepsis is a significant infection-related health problem that causes dysregulated host responses with potentially fatal organ dysfunctions. There are performance gaps for sepsis in our intensive care unit (ICU), so it is an issue in our unit because it is associated with a high mortality rate, especially for late diagnosis and treatment. It causes cognitive impairment, leads to organ damage, and neurological problems, increasing the risk of death.

ICU bedside nurses and physicians will complete the routine early detection procedure (risk assessment and evaluation) for all ICU patients at admission. The short-term outcome of implementing this strategy will be reduced sepsis-associated complications and fallouts. The data collection plan will be to access patients with potential sepsis risk, run the sepsis bundle protocol, and then see where fallouts are. After recognizing the fallouts, then educate to prevent fallouts.
