Palmetto Patriots Home is committed to providing Veterans with a continuation of their active and enriching lives after active military service. Social Workers work with the residents, family, and multidisciplinary/care plan team to develop individual plans of care. A plan of care covers medical care and services, daily living activities, and educating the residents to maintain or increase independence to improve their well-being. Care is provided 24 hours a day, seven days a week, to all residents. A fully staffed social work department offers counseling, assistance in obtaining benefits, financial and other important services to residents and their families.

Eligibility for admission into the Palmetto Patriots Home is limited to three main criteria. Veteran served active duty with an honorable discharge, Veteran has been a resident of South Carolina for the previous 12 months, Veteran meets Veterans Administration criteria for skilled or intermediate nursing home care.

My role as an advanced practice intern is interesting. So i have to do assessments daily on the clients. Initiate psycho-social evaluation, enter MDS interview information, evaluate, assess, and update plans of care, oversee family meetings, investigate any abuse/neglect allegations at a 104 bed skilled nursing veteran facility. Oversee social services department. Crisis and case management for veterans. Coordinate in facility medical clinics. There is no difference in my prior field placements. I'm aware of a lot of the policies at this agency which makes this field placement easy.
