The patient Ill appearing 50yo gentleman presenting with sharp cramping abd pain with bloating, nausea and vomiting x 3 days. Hx of divercutilitis requiring hospitalization. Denies alcohol abuse. Reports anorexia and weight loss. Pt denies ability to pass stools or gas. Denies blood in vomit or dark stools. Pt was dx w/ Large bowel obstruction.

Treatment:

Refer to ER for management of LBO d/t mass of transverse colon and hypokalemia.

IV fluid for rehydration.

Antibiotics should be started in the emergency department. Coverage must include gram-negative aerobic and gram-negative anaerobic organisms. IV metronidazole with intravenous ertapenem effectively reduce infectious complications in elective colorectal surgery.
