Problem: LL is a 33 y/o female who presents to your clinic with asthma and seasonal allergic rhinitis each spring. She describes her asthma as "mild." Except during the spring, her daytime symptoms occur less than once/week and she does not have nocturnal symptoms. Each spring, her symptoms worsen, and she requires albuterol 3-4x/day. During springtime she Albuterol and ipratropium were given by nebulizer with little improvement. Oxygen at 4L/min by NC was started, followed in 20 minutes by a second dose of albuterol. Subsequently, the patient's HR increased to 140, and he c/o palpitations and shakiness. What adverse effects is HT experiencing that are consistent with/ beta-agonist administration? Why did HT fail to respond to the initial therapy? Could tolerance to the beta-agonists have contributed? A nonprescription antihistamine which offers some relief. How could LL's management be improved?
