Case Study: Nurse Keesha has worked at XYZ Hospital for 12 years. She was shocked to hear that a new policy going into effect next week includes mandatory delegation of all vital signs to the certified nurse assistants (CNAs), who are to perform them 3 times on each patient each shift. She knows the CNAs with whom she works are already challenged to complete the tasks they have each shift. She thinks vital signs performance on each patient three times each shift is unrealistic if the CNAs are to keep up their other duties.

Keesha knows that if the CNAs attempt to follow the new policy, other tasks will not be performed. Keesha thinks that checking vital signs once per shift is sufficient for most patients unless they are having problems. That is how often the nurses had been required to check vital signs when it was their task.

Questions:

  1. How could Keesha find out the rationale for the change in policy?
  2. With whom should Keesha discuss the new policy?
  3. What should she do?
