Problem

A Hispanic woman, Mrs. Rivera, had to sign an informed consent form for a hysterectomy. The patient spoke no English and the hospital staff relied on her bilingual son to serve as the interpreter. When the son explained the procedure to the mother, he appeared to be interpreting accurately and explaining the proper procedure. His mother signed the consent form willingly. The day after surgery, however, when a professional interpreter visited Mrs. Rivera, she learned that her uterus had been removed and that future pregnancies were not possible. Mrs. Rivera became very angry, threatening to sue the hospital. Explain where the break in the chain of communication in this scenario occurred.
