As a new APN, I'd like to take the lead in lowering stroke patients' readmission rates. Stroke is a major cause of long-term disability and death. Stroke death rates in the United States are declining, but projections indicate that prevalence will continue to rise due to an aging population. An additional 3.4 million adults in the United States will have had a stroke by 2030, a 20.5% increase from 2012. Stroke disproportionately affects Blacks; African Americans are twice as likely as Whites to suffer from a first-time stroke, and Hispanic death rates have been rising since 2013. Every year, hundreds of thousands of stroke survivors are born, many of whom suffer from a variety of poststroke disorders, which can lead to readmission. How can we decrease the re-admission?
