For this assignment, you will use your team's research question from Team Assignment 1 to assist you with individually creating a survey questionnaire. Although you will develop this assignment on your own, you will collaborate next week to combine the individual surveys into one comprehensive survey.

In an MS Word document, include the following items.

- Introduction - Use your team's research question to guide the survey. Provide an introduction to describe your team's research topic along with context learned in your literature search.

- Create a Survey Questionnaire:
- Based on your refined research question and using the readings to guide you, develop a set of questions that will help you collect the information you will need to answer your research question.
- The survey should be composed of a variety of question types, including but not limited to, Multiple-Choice, Single - Response Scale, Multiple-Choice, Multiple-Response Scale, Likert Scale Summated Rating, and Numerical Scale.
- Use the Westeliff University Survey Question Guidelines below to create your survey questions.
- Under each question, include a one sentence statement USING ITALICS which explains why you recommend each question be added to this survey.
