Phase 6-Implementing the Intervention

• Have documentation printed and ready at the start of the implementation
• Have the necessary funds allocated for starting the process
• Do the community outreach and gather a group of people that are willing to participate in the program.
• Gather health information, including vital signs, weight, BMI, and Hba1C, from the group participating in the program.
• Schedule meetings with participants and start the educational progress with participants involved.
