An 82-year-old patient with a history of moderate dementia has been hospitalized with a diagnosis of pneumonia. the report from the assisted living facility states that the client had lunch a few hours ago with his wife at the AL facility, and had started coughing while eating his desert.
A few hours later, the client was sitting in his room when staff walking by the room saw him suddenly stand up and start to undress himself.
When the staff intervened, he became physical, striking out at staff member and yelling. Staff from the assisted living facility reported that normally the patient is pleasant and alert with occasional periods of confusion, but has been easily redirected.
Vital signs: Temp 103.2, Respirations 25, Apical HR 100, Blood Pressure 129/82.
The patient is now disoriented to time, place and self.
