Your employer, Maryville General Hospital, has asked you to draft a new hospital-wide policy and procedure on informed consent (for both the hospital-owned medical practices as well as the hospital itself). Before you begin drafting, you go to your supervisor and pitch her on what you believe should be included in the policy and procedure.

  1. In what form should consent be obtained? Are there exceptions?
