Hospital is the patient med reconciliation section

Question

One section that needs to be updated in my hospital is the patient med reconciliation section. The med rec is to be done at each admission. An improvement would be showing when a medication was placed in the med reconciliation because this does not update unless the nurse changes it. Some patients have not been to the hospital in years, so the listed meds are sometimes not used anymore. If we could see the last time a medication was entered, it would indicate if the medication is possibly one that is no longer used. Some patients do not remember when medicine was prescribed, so if I say you told us about taking lisinopril in 2005, they may remember that they were switched off that medication a while ago. This happens a lot with medications. This would save time.
