Question

Mike Adams, a 42-year-old Caucasian male, is admitted to the emergency department from home with a complaint of shortness of breath. He has a history of hypertension and well-controlled diabetes. Upon presentation to the emergency department, his vital signs are: temperature, 101.9°F (38.83°C); pulse, 120 bpm; respirations, 20; blood pressure, 90/54 mm Hg, and SpO2 91%. His chest radiograph reveals pneumonia. He is alert and oriented, but he complains of dizziness when changing positions. His weight is confirmed at 80 kg/176 lbs. The following orders are written for the patient: Place 2 large bore IVs and infuse a 2.5 liters (30mL/kg) bolus of 0.9% normal saline then begin 125 mL/hr Obtain complete blood cell count, serum electrolyte level, lactate level, and blood cultures Provide oxygen at 2 L/min via nasal cannula Begin administration of cefotaxime 2 g IV every 8 hours after blood cultures are drawn.
