Patient is and 89 year old female who presents with a history of fall at home resulting in inability to walk and groin pain. They present complaining of pain in the hip which is constant, worse with movement and better if held still. The injury occurred 24 hrs ago. There was no other known injury, no chest pain, no shortness of breath, no loss of consciousness and not other extremity injury. Under the nursing priority concept of impaired mobility the nurse came up with the following SMART #1 Outcome Statement: The patient will demonstrate the ability to reposition in bed independently and ambulate 10 feet using a walker with minimal assistance within 48 hours post-surgery, as evidenced by improved muscle strength and absence of pressure sores. the patient not able to successfully repositioned themselves in bed independently, observed through direct clinical observation. The patient was not able to ambulate 10 feet using a walker with minimal assistance, verified by a nurse so goal wasn't met. What would you do differently in your care next time to assist the client in meeting this outcome?
