Question

An older person will need an aged care assessment if they have complex aged care needs and want to: Access aged care services through any level of Home Care Package Receive services through transition care Receive respite care in a nursing home Move into a nursing home. An aged care assessment is conducted and must be approved by an Aged Care Assessment Team (ACAT), also known as an Aged Care Assessment Service (ACAS) in Victoria. To conduct an assessment, an ACAT member (normally a nurse, social worker or other health care professional) will arrange a visit to the older person's home (or place of residence/accommodation) and discuss the older person's needs and the current management of their day-to-day life with the older person (and in some cases with a carer or other relevant person). To ensure that all clients in the aged care industry are receiving the right care for their needs, they need to be assessed on their eligibility for care.
