CASE: Patient name John a 69-year-old man with a 2-year history of type 2 diabetes. has been referred by family members to the Diabetic Clinic for thorough assessment to come up with a plan to control his diabetes. Patients present with recent weight gain, suboptimal diabetes control, and foot pain. Prior to diagnosis he already had symptoms of hyperglycemia and family history (mother) of Type 2 diabetes. The patient's current weight is 218 lbs. and has a BMI of 31.3 (OBESE). He had a history of 22 lbs. of weight gain from the past year which is 24% of his UBW. Vitals show 140/90 blood pressure indicates stage 2 high blood pressure and Fasting capillary glucose reading of 166 mg/dl indicator of diabetes. Lab results 15 days prior to the office visit show elevated fasting glucose 178 mg/dl, elevated level of triglycerides at 177 mg/dl, elevated level of A1C at 8.1%, and elevated level of Urine microalbumin at 45 mg - which all supports TYPE 2 diabetes.
