Mr. V is a 70-year-old patient in an acute-care facility in Dallas, TX. He is recently widowed and has a history of stroke and COPD. He is also a two-person assist with his mobility. He is a man of routine and always wants to do things his way. Staff are aware of him displaying difficult behaviors, such as swearing and hitting, when things do not go his way. One day, Dylan and Diana, two RNs on the unit, arrived 10 minutes late to Mr. V's room for his 7:45 morning care. He immediately shouted for a urinal. Dylan rushed to get the urinal, but Mr. V had already started wetting his bed. He disappointedly told both nurses that no one had come to attend to his needs. Diana responded to Mr. V by stating that he should have used the call light sooner instead of waiting for staff to arrive. Mr. V started closing his fists and breathing fast. He then shouted at both nurses, calling them stupid, and because Dylan was closest to Mr. V, he started hitting him. Diana immediately began to make her way toward the door and stated that she was not there to take on.
