Question

The parents of a 5 month old infant brought him to an urban hospital emergency department because of a five day history of "spitting up, diarrhea and poor feeding." HX: The child had been healthy until the onset of his present problem. No other family members were ill. He had vomited approximately three times and had approximately ten liquid bowel movements in the previous 24 hours. Infant is strictly breastfeed on demand averaging 6-8 feedings in a 24 hour period. His mother stated that he was not having as many feedings, with only 4-6 feedings over the last 2 days, however she assumed he was beginning to wean. The mother has not pumped so cannot estimate how much milk is expressed with each feeding, however the infant has shown good weight gain since birth. Upon direct questioning it was learned the infant had not wet his diaper in the previous hours and only two times in the previous 24 hours. PMHX: none, NSVD at 38 wks, apgars 9/9, birthweight 3.2kg Medication History: none Family HX: none Social: Infant lives at home with mother and father and does not attend daycare. Mother is a stay at home.
